Out of Hours
Out of hours cover is provided by NHS England between 6.30pm and 8.00am and from 6.30pm Friday until 8.00am Monday. Patients will be offered appropriate advice or a consultation at Savernake hospital, if medically necessary a doctor will make a home visit. Telephone 111

The doctors and nurses are available for telephone advice. Please ring in the morning before 10.30am if possible, and the doctor or nurse will return your call

Cancelling your Appointment
If you are unable to attend an appointment with one of the doctors or nurses, please telephone  to cancel your appointment. By giving us as much notice as you can you are helping us to make sure that someone else is given your slot.

Registration Form

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will need to sign this form to confirm that the details are correct.

Alternatively you may print off a registration form, fill it out, sign it, and bring it in with you on your first visit to the practice.

If you are on any repeat medication, please make a GPs appointment when you first register. If you wish, you can also arrange a New Patient Check with one of our nurses who will go through your medical history and address any health problems you may have.
