Out of Hours
Out of hours cover is provided by Wiltshire Medical Services between 6.30pm and 8.00am and from 6.30pm Friday until 8.00am Monday. Patients will be offered appropriate advice or a consultation at Savernake hospital, if medically necessary a doctor will make a home visit.

You will be offered a ‘New patient check up’ where we will go through your medical history and address any health problems you may have. You will also be offered basic screening such as blood pressure and urine testing.

PreRegistration If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
