Burbage Surgery provides patients with the online facility to: Order repeat medication / Book appointments directly with our nurses or doctors / View their Summary Care Record / And upon written request, gain access to their detailed coded medical record.
You will need to obtain your registration ID and password for on line access. Please contact the surgery to enable this service. Once you have these details click on the link below to start using the service.
We have made some appointments available with GPs, and phlebotomists for patients to book on-line. Please note these are only 10 minute appointments. You can still telephone the surgery directly for an appointment if the times of the on-line appointments do not suit you or if you think you will need longer than the allocated 10 mins. Nurses appointments are currently pre-bookable for blood tests only: other procedures may need longer than the allocated 10 minute slot
If booking an appointment for a blood test, please note that this appointment needs to be made in the morning or before 3:30pm thank you.
For those patients wishing to gain access to view their medical records on line you will need to complete the Application for online access to my medical record form. This can be collected from surgery and proof of identification will be required to process your application.
